WinCon Tattoos: Go Ink Yourself

ATTENTION wincon PEOPLE!

It's about two months from the con right now, and today me and tsukinofaerii are gonna be checking out tattoo shops for everyone. Because of this, I need the following information from everyone getting a tattoo with us or who is even thinking about getting a tattoo with us.

If you do not fill this out, I may not be able to get you fit in for the appointment. The sooner you can fill this out, the better.

Brief description of what you might be getting tattooed: A tiny Firefly, floating on an updraft with a few leaves.
Picture reference (if you have one): I'm planning on editing it slightly, changing the font and some of the leaves, maybe adding some color, but here's the basic template: http://oi55.tinypic.com/30t3z2x.jpg
Where you are thinking about getting it tattooed: On my left arm, under Atlantis.
What size tattoo you are planning on getting: roughly 3-4 inches tall/whatever that equates to wide.

Brief description of what you might be getting tattooed: Small sewn heart (red thread) with a needle sticking through it.
Picture reference (if you have one): My first draft didn't have the needle going through the heart - I'll be editing it, but here's a rough idea: http://oi51.tinypic.com/5kplia.jpg
Where you are thinking about getting it tattooed: On my upper chest, just below and to the right of my throat, below the collarbone.
What size tattoo you are planning on getting: Small, maybe around 2in x 2in.
